Dads has published a Provider Letter regarding the deadline and process to submit a request for partial reimbursement of a fire sprinkler system in a Four-Person Residence for HCS providers. Refer to the letter for what is required to be in the request and how to submit it.
Note that the request must be emailed by 5 PM on June 30, 2017.
