DADS is conducting a review of all forms used across DADS waiver programs as part of an initiative to standardize and streamline. Updated versions of the forms included in the Phase IV review are available. An excerpt:
Form 3599, Attendant Orientation/Supervisory Visit, and Instructions
Revisions to this form include re-organization of the signature section, and updating the plan of care tasks. In addition the following information fields were added:
• Emergency Contact;
• Second attendant schedule and revised schedule format;
• “Is the attendant competent to provide medically related tasks?” question;
• “Describe attendant training needs;
• “Describe corrective actions taken;” and
• Additional comments.
CBA, CLASS, MDCP, PHC, CAS, FC, and CDSA:
In a letter than pertains to providers only in Regions 2 and 4, DADS determined that for February and March 2012, it will accept paper timesheets as a back-up to the Electronic Visit Verification system to document time attendant hours. (Letter)
