CLASS, CMA, and DSA
DADS will review all forms used across DADS waiver programs, including CLASS forms used by CMAs and DSAs, which will be reviewed and revised in phases. Stakeholders will have a chance to review revisions before forms are updated. (Letter)
— CMS has posted the “November 3, 2011 National Provider Call Follow-Up and Clarifications” on its website at this link. DADS urges providers to read the document to help complete the assessments correctly.
— DADS has posted a revised form 3621R, Class Individual Plan of Care, here: CLASS Forms Table of Contents.
— The DADS Consumer Rights and Services section will pilot an expansion of Call Center hours. Intake staff will be available to take live calls Monday through Friday from 7: a.m. to 7 p.m., to evaluate full service delivery during those hours. The pilot is effective January 2, 2012, and will end on June 30, 2012.
— The Medically Dependent Children Program waiver is available for review in this PDF document. A guide to reviewing the waiver application can be accessed here.
— The CBA program waiver is available for review in this PDF document. DADS will hold a stakeholders meeting about the renewal on January 3, 2012, from 9:30 a.m. to 4 p.m. at the John H. Winters Complex, 701 West 51st Street, Austin. See this entry’s section for details.
