- DADS has updated the DBMD Program manual. Effective July 26, 2010, the following DBMD forms and instructions have been updated:
- Individual Plan of Care (Form 6500)
- IPC Service Delivery Transfer worksheet (Form 6500-T) (formally ISP Service Delivery Transfer worksheet)
- DBMD Summary of Services Delivered (Form 6503)
- Prior Authorization for Dental Services (Form 6504)
- DADS will host a provider meeting for Region 6 HCSSA providers on August 11, 2010, from 1:30 p.m. to 5 p.m. at the United Way of Greater Houston, Auditorium, 50 Waugh Drive, Houston, Texas 77007.You may register here.
- DADS will host a provider meeting for Adult Day Care/Assisted Living providers on September 16, 2010, 1 p.m. to 4 p.m. at the United Way of Greater Houston, Auditorium, 50 Waugh Drive, Houston, TX 77007.You may register here.
- DADS will host one-day forums that focus on community employment for individuals with intellectual and developmental disabilities. Registration for the free forums in San Antonio on September 14 and in the Rio Grande Valley on September 16 closes on September 3.
