The Texas Department of Aging and Disability Services (DADS) released two information letters:
- Billing Procedures for CMA Vendor Number Transfers
In a letter to Community Living Assistance and Support Services (CLASS) Case Management Agencies (CMA), DADS clarified billing procedures for CMA vendor number transfers, which will be handled the same way as other transfers, except that two 3625 forms must be completed when transfers occur within a month. - Revisions to Forms 3621 and 3621 – T
DADS reminded CLASS Providers that it revised Form 3621 (Individual Service Plan) and Form 3621-T (ISP Service Delivery Transfer Worksheet) to reflect changes to the respite service codes for individuals participating in the Consumer Directed Services model. The new codes are Respite In-Home (11PV) and Respite Out-of-Home (11AV). Effective June 1, case management agencies must use the new forms for upcoming Reassessment ISPs and Initial ISPs.
